To save time during the first visit, please complete the initial appointment forms to bring to your first appointment. The HIPAA Notice of Privacy Practices is for your records and does not need to be printed.
Your appointment time is saved specially for you. If you need to reschedule your appointment, please notify me a minimum of 24 hours in advance. If you cancel within 24 hours of your appointment, you will be charged the full cost of the session.
